Pre-Hospitalization: To make an inpatient appointment, please stop by the Medical Support Area on the 2nd floor of the Advanced Medical Center after your outpatient consultation. We will make an admission reservation and explain about the examination.
If you have made an appointment for hospitalization, the hospital will contact you at a later date with the date and time of your admission.
Once your have received notification of your admission date, please come to the hospital on the appointed date and time. Submit the following documentation to admissions reception (Admitting & information desk on the second floor of the outpatient clinic) before going to the ward. (Reception is open from 8:30 to 10:00 a.m.)

Health Insurance Card ,*My Number Card as your Health Insurance Certificate, Latter-Stage Elderly Medical Care Certificate: ・Checking and Updating Your Health Insurance Information On the day of your admission, your health insurance documents (e.g., Health Insurance Card, Eligibility Certificate for Ceiling-Amount Application or any other insurance-related documents, if applicable) will be checked at the Inpatient Reception Desk to confirm your eligibility. All documents will be returned to you immediately after the confirmation. If there is any change in the information on your health insurance card, or if you lose your eligibility for your health insurance plan, please notify the Inpatient Reception Desk immediately.Patient registration card

Patient Registration Card ・Your patient registration card will be collected by the ward when you are admitted and returned to you upon your discharge.

Other ・If you are currently applying for, or wish to apply for, medical and welfare-related benefits (such as childcare, rehabilitation, or foster care) or workers' compensation, please inform us during your admission procedures. ・We also ask that you notify us if you are being admitted due to injuries from a traffic accident. ・If your insurance card information changes, or if you lose eligibility during your hospital stay, please be sure to inform the Inpatient Reception Desk (2nd floor, Outpatient Building), even if it may be inconvenient. ・If you have nail art on your fingernails or toenails, please remove it before admission. ・If you need to change or cancel your admission appointment, please contact your ward or the Inpatient Reception Desk in advance.
