Admission Processing

• If you have made an appointment for hospitalization, the hospital will contact you at a later date with the date and time of your admission.
• Once your have received notification of your admission date, please come to the hospital on the appointed date and time. Submit the following documentation to admissions reception (counter 3 of the Patient Support counter on the second floor of the outpatient clinic) before going to the ward. (Reception is open from 8:30 to 10 a.m.)
• At admission processing, please submit the following documentation to admissions reception (counter 3 of the Patient Support counter on the second floor of the outpatient clinic).
  1. Insurance card
  2. Patient registration card
  3. Admission insurance card
  4. Hospital gown loan request form
