To First-time Patients

• Obtain a "Consultation Request Form" from the General Information counter, fill in the necessary fields within the red borders, submit it to counter (1) at the Patient Support counter, and wait to be seen. We will register your information, and call your name.
• Check the department you wish to have a consultation with, and hand over your patient registration card.
• Please ask at the General Information counter if you do not know which department you should go to.
• Please submit your patient registration card at the department where you are to have your consultation.
