  • Be sure to bring your patient registration card, My Number Card as your Health Insurance Certificate, a recipient card for elderly medical care, and medical care entitlement card, etc. when visiting for a consultation.
    You may be required to pay all costs yourself if you do not present your insurance card, medical care entitlement card, or other relevant documents.

  • For treatment related to traffic accidents or work-related injuries, please notify the staff at window (1) or (2).

  • Regarding medical treatment for patients traveling from overseas:

    Please contact us through a medical coordination company that has a business partnership agreement with our hospital if you are seeking medical treatment from overseas. Please note that coordination fees will be borne by the patient. We appreciate your understanding and cooperation.

  • 1. Disaster Preparedness and Evacuation Routes for Outpatients
    ・In the event of an emergency such as a fire or earthquake, first ensure your own safety, check the emergency and guide lights as well as evacuation routes around you, and follow the instructions of doctors, nurses and hospital staff.

    Never use elevators, as they may stop between floors due to a power outage, etc.

    2. Disaster Preparedness for Inpatients
    ・From the Admission Guide 19 :
     In Case of an Emergency 
     ①When you are admitted to the hospital, please be sure to check the locations of emergency exits, etc.
     ② In the event of an emergency such as a fire or earthquake, please follow the instructions of doctors, nurses, and hospital staff.
     ③ Never use elevators under any circumstances. 
     * In Case of Fire
     In the event of a fire, please follow the instructions of staff and evacuate promptly.
     * When an Earthquake Alert is Issued
     ① The wards are equipped with earthquake-resistant structures, so you are safe. Please remain calm.
     ② If you are away from your hospital room, please return immediately.
     ③ If you wish to go home, please consult your doctor.
     ④ To protect yourself, please follow these precautions:
      ・ Do not go near windows.
      ・ Be careful of breakable objects.
     * In the Event of an Earthquake
     (1)Do not rush out of your hospital room in a panic.
     (2)Anticipate falling objects and protect your head with a blanket or similar item.
