　"Hamamatsu Regional Support for Cardiovascular Disease" was established in April 2025, based on the Japanese National Plan for Promoting of Measures against Cerebrovascular and Cardiovascular Disease. Our aim is to overcome cardiovascular disease and extend the healthy lifespan of Hamamatsu residents. In response to the rapid increase in cardiovascular disease patients (especially heart failure), we seek to develop a comprehensive cardiovascular care support system, which includes seamless regional cooperation across the continuum of care: from emergency and acute phases to chronic care, home medical service, and long-term care throughout Hamamatsu city.
