Foreigners who will stay in Japan for over 90 days need to register as an alien at the municipal office in the municipality that you reside within 90 days of entering the country. An alien registration card will be issued when the registration is completed. Foreigners are required to carry this card at all times.

For a new registration, fill out an alien registration form which is available at the municipal office and submit it with two copies of your photograph (4cm X 3cm), your student identification card, and your passport.
You need to apply for changes to your registration within 14 days, if any information on the certificate of alien registration such as "address", "name", "nationality" or "visa status" has changed. To register the changes, fill out the alteration registration form which is available at the municipal office, and submit it with your Alien Registration Card. In addition, you may be requested to submit the documentation to verify the changes as needed.
If you loose your Alien Registration Card, you have to apply to reissue the card within 14 days of your knowledge of having lost it. You can also apply to reissue the card when it is notably damaged.
If you intend to stay in Japan longer than the valid date of the card, you have to apply for a renewal at least 30 days prior to the expiration date. To apply for a renewal, fill out the alien registration renewal form which is available at the municipal office and submit it with your passport and 2 copies of your photograph (4 cm X 3cm).
When you leave Japan (except in the case that you have a re-entry permit), you need to surrender your alien registration card to the officer at the immigration office in the port of departure from Japan.
